Concrete Performance Survey

Privacy Notice

Effective September 5, 2026

Information collected

The survey collects the information you choose to provide about you, your company, a concrete project, reported performance concerns, communications, financial impact, and supporting documents, media, or evidence-sharing links.

How information is used

Information is used for follow-up, verification, case coordination, evaluation of reported concrete-performance concerns, and aggregate analysis. Public results are presented without names, contact information, or project addresses.

Access and storage

Identifiable submissions are limited to authorized survey administrators. Survey responses are retained in the survey database and response spreadsheet, while uploaded files are stored separately with the submission. Information is retained while it remains reasonably necessary for the purposes above.

Anonymous traffic measurement

The survey records privacy-limited usage events so the project team can measure page views, approximate unique visitors, survey starts, submission attempts, successful submissions, and failures. These events use random browser and session identifiers plus the date and time, referring website hostname, page path, and broad device category. Names, email addresses, survey answers, and uploaded evidence are not stored in the traffic-measurement record.

Your choices

Submitting the survey is voluntary. You may ask a question, provide supporting documents later, request a correction, or request removal of your submitted information by emailing Type1lconflict@gmail.com. Include your company and project name so the correct submission can be located.

Important limitation

The public statistics describe reported survey responses and documented conditions. They do not, by themselves, establish causation or constitute a scientific or legal conclusion.
